I have read and agree to all of the following:

1. I understand that in order to participate, I must surrender the entire litter to the shelter and cannot adopt out any of the animals prior to using the service.

2. I understand that if I utilize free veterinary services through the shelter for this program, I must leave my credit card information and could be charged a $100 fee if I do not return the litter to the shelter after receiving services.

3. I agree to allow the shelter to spay mama and neuter dad as a requirement of this program.

4. I agree to work with shelter staff to schedule spay/neuter surgeries depending on availability.

5. I agree to foster the litter until they are eligible for surgery (8 weeks old).

6. I agree to keep the litter safe and healthy during my time of foster. This includes keeping all animals indoors and limiting exposure to other animals.
