General Release of Liability

1. I release any and all staff, volunteers, officials, officers, insurers, or any other individual affiliated with the WVHS from any and all civil liability and/or any and all forms of injury which may arise as a result of community service or volunteer work while in and on the grounds of WVHS.

2. I also acknowledge that there are many dangers and risks associated with community service/volunteer work at the animal shelter and I am granting that I do hereby release any and all persons affiliated with WVHS from any injuries, whether physical or mentally sustained by me or the individual I have guardianship over.

3. Tetanus Information: I understand that because I, or my child (if applicant is under 18), may handle animals, it is important to discuss being vaccinated against tetanus with my physician. I release WVHS from all responsibility that may occur because of my not pursuing this matter further and I understand whatever decision I make is at my own risk.

4. In the event of an emergency, I hereby give WVHS permission to seek medical attention for myself or my child, if the applicant is under 18.

5. I give permission for WVHS to photograph me, or my child (if the applicant is under 18), for use in any publication or advertising purposes that WVHS may designate. I acknowledge and understand that as a volunteer of WVHS, I or my child (if the applicant is under 18) is not covered by WVHS workers compensation or any other insurance policy for any damages or injuries I, or my child, may sustain during volunteer activities.

6. I understand that WVHS activities may involve strenuous activity on my part. I am in good health and am aware of no physical problem or condition that will limit or interfere with my ability to participate in the activities I have agreed to. I have read and understand this agreement and am of legal age. I am legally competent and am signing this document of my own free will, devoid of any influence of a WVHS staff member.

7. I, by my signature below, acknowledge, understand, and agree to all of the previously stated terms and acknowledge that I may request a copy of this agreement.

Minor Release of Liability

1. I am the legal parent/guardian of the child/children mentioned above in this application.

2. The release of liability agreed to as part of this application holds true for minors that are volunteering with me.

3. I understand that there are risks associated when working with animals and that safety should be the highest priority at all times.

Photo and Media Release

1. I give permission for WVHS to photograph me, or my child (if the applicant is under 18) for use in any publication or advertising purposes that WVHS may designate. I acknowledge and understand that as a volunteer of WVHS, I or my child (if the applicant is under 18) is not covered by WVHS workers compensation or any other insurance policy for any damages or injuries I, or my child, may sustain during volunteer activities.
