I have read and agree to all of the following:

1. I certify that I am 18+ years old.

2. I certify that all information I have given on this form are true and correct.

3. I understand that WVHS will likely not be able to provide me with the brand of food that I normally feed my pets.

4. I understand that by participating in this program I am agreeing to all stipulations set by the shelter.

5. I agree that if any of my animals are currently unaltered, I will have them altered within 3 months of participation. I understand the shelter can help if I email them at help@westvalleyhumanesociety.org.

6. I understand that this food is being given to me and it is unlawful to attempt to sell it.

7. I agree to indemnify and hold harmless West Valley Humane Society, the attending veterinary, veterinary technician, any other WVHS employees and/or volunteers from any and all liability arising from the shelter assisting me with food for my pets.
