I have read and agree to all of the following:

1. I agree that I am over the age of 18 and that this is not my personal cat.

2. I understand that it is my responsibility to release this feline in the exact location where I found him/her.

3. I accept the animal at my own risk and agree that no promises or guarantees have been made to me regarding the health of the animal, nor its behavior, age, breed, or habits. I also understand that this animal may have been exposed to, be carrying, or have a contagious disease.

4. I agree to provide the animal with humane care, adequate food, water, shelter, and kind treatment at all times until I release the feline. I agree not to allow the animal to be used for medical or any other experimental purposes.

5. I understand that if a female animal has an abdominal scar, but was not spayed at West Valley Humane Society (WVHS), that WVHS cannot know for certain if the animal has been spayed. I understand that the cat will be ear-tipped.

6. I understand that the participating cat(s) are not covered by WVHS for any medical treatment. If the animal has or may have any medical condition(s) that may require additional veterinary consultation or treatment, it is my responsibility to seek outside help and support.

7. I understand that for any reason West Valley Humane Society may choose to keep cats for their adoption programs.

8. I consent to and authorize the use of anesthetics, other necessary medications, and the performance of sterilization surgery, commonly known as spay or neuter surgery.

9. I agree to indemnify and hold harmless the West Valley Humane Society, the attending veterinarian, and any of the officers or employees of said entity from any and all claims, damages, and causes of action that may arise from the procedures or operations to be rendered, and from other medical care arising therefrom.

10. I agree to hold West Valley Humane Society harmless for any damages or injuries that may arise from the animal. I further agree and understand that WVHS cannot predict how an animal will behave nor can WVHS be held responsible for animal's behavior. I understand that should an injury occur to any person or another animal from this animal, WVHS is not responsible for medical payments or debts incurred and cannot be held liable.
