I have read and agree to all of the following:

1. WVHS offers no-cost, limited, basic examination to adopters within 14 days of adoption and for animals in our foster home.

2. WVHS has limited staff and resources and resources and can only offer a basic physical examination to address minor concerns including "kennel cough", feline upper respiratory infections, or spay/neuter incision checks.

3. Adoption of an animal includes taking responsibility for the pet's health and well-being dating from the time of adoption.

4. WVHS lacks the resources to provide diagnostic tests including radiographs (x-rays), blood tests, cytology/biopsy, fungal cultures or bacterial culture and sensitivity. If you feel that your pet requires diagnostic test, please see your regular veterinarian. WVHS may recommend that you seek diagnostic tests at your regular veterinarian.

5. The post-adoption recheck is not meant to replace regular care or examination with your veterinarian.

6. Adopted pets may have previously undiagnosed medical conditions. WVHS is not responsible for diagnosis and treatment of previously undiagnosed medical conditions.

7. Some conditions may be beyond the scope of the post-adoption recheck to address, in which case WVHS will recommend that you make an appointment with your regular veterinarian.

8. Animals at WVHS may have been exposed to bacterial, viral, protozoal or parasitic infections.

9. Availability of post-adoption rechecks is limited based on clinic staffing. If you feel your pet needs immediate or intensive care, please see your regular vet.

10. Public spay/neuter clinic clients are limited to a paid examination. Additional care will be for a fee to include medications, surgical corrections due to not following post-operative instructions, etc. If the care needed exceeds the resources available, you may be referred to your family veterinarian.

11. I agree to indemnify and hold harmless West Valley Humane Society, the attending veterinary, veterinary technician, any other WVHS employees and/or volunteers from any and all liability arising out of the performance of all procedures during my visit.
