I fully understand that the West Valley Humane Society has the right to refuse to perform this procedure if, in the opinion of a WVHS staff veterinarian, euthanasia is not in the best interest of the animal. If euthanasia is declined, I will not be contacted, and I understand the animal will become the property of the West Valley Humane Society and may be adopted or transferred to another agency. The euthanasia fee will be given to the shelter for relinquishing the animal. I authorize the West Valley Humane Society to dispose of the remains in accordance with the WVHS policy. There is no additional fee for this service.
