The following application is for requesting animal cremation services. Pricing rates for Cremation can be located under the Pricing page. This form is for cremation services only (no euthanasia needed) for pets who have already passed and need to be cremated. Please be prepared to upload a photo of your pet and your drivers license or government issued ID.
